The PayersAs Medicare and the QIOs develop strategies to decrease payments for "medically unnecessary" services and over coding the payers will start to do the same. Though they have been slow to embrace the Observation Status concept they will begin to encourage non-inpatient treatment of diseases characterized by short inpatient stays. They recognize that they have been over paying for patients admitted with chief complaints of "Chest Pain", "Abdominal Pain" and "Asthma". The payers are developing payment strategies to encourage Observation Status and it will behoove innovative hospitals to develop a comprehensive and financially viable Observation/Geri-Care Strategy.
