The HospitalHospitals may be the biggest financial victim of the national EDO crisis and they are probably the single largest contributor to this problem. In general the hospitals have taken a "laissez faire" attitude with the admitting practices of their attending physicians. By allowing the attending to consume hospital resources (beds) in an unchecked fashion they are relinquishing control over the hospital's financial viability. The admitting physicians will always tend to admit patients to a higher level of resource (telemetry and units) than is clinically warranted and supported by resource management tools (Interqual and Millimen). This lack of management and accountability forces the hospital to spend more resources (cost) per patient than the reimbursement allocated (payment). The overuse or inappropriate use of inpatient resources contributes to EDO in a vicious cycle where the limited availability of inpatient beds generates more patients waiting for admission in the ED thereby limiting the beds in the ED to treat sick patients.
